- A set of 55 full tubs had been drawn by main and tail rope haulage into a siding, and was being lowered back on to the mooring chain when a centre bar crook broke, and 53 of the full tubs ran amain down the siding. The deceased was at the rear end of the set, and had knocked off the tail rope and was probably holding up the "cow" whilst the set was being lowered back. There were three refuge holes within a distance of 10 yards, but apparently he had not time to reach one. The first seven tubs passed him all right, but the next got off the way and he was crushed by it. There was a slight flaw in the crook at the shoulder close to the tub which would be difficult to detect.
