He was sitting upon the brake lever of a self-acting incline wheels, to brake the set of full tubs which was being sent off from the top. As soon as the set moved the capple or socket at the end of the rope caught on the end of the bolt which coupled the lever to the brake strap and the lever was thus violently jerked to the roof. The lad slid of but was struck by the rising lever and killed.
