Ashurst, who was in charge of the surface screening arrangements, appears to have gone under a fence rail, 3 feet 6 inches in height, for the purpose of examining the machinery and belting whilst it was in motion, with the result that some portion of his clothing got caught in the key of a revolving shaft and he was whirled around and fatally crushed.
