- Six empty tubs were being clipped to the rope in a short rise drift. When the tubs were 15 yards up the clip slipped and the tubs ran back dashing into some empties at the bottom and derailing them. Ashton, who was about to enter a refuge hole, was struck by some of the tubs and fatally injured. Retaining catches were provided, but they were of poor design and failed to prevent the accident.
