- This accident was caused by a tube containing a haulage rope piercing the top of the cage in which deceased was ascending at the end of his shift. There had been a movement in the shaft near the Six feet landing which had resulted in the walling being crushed out of perpendicular and I think it had forced the tube outward until the lower joint broke and a length of tube hung diagonally in the shaft. On inspection being made the tube was found to be touching the walling a few feet above where the accident occurred.
