A tram of rubbish had been pulled a certain distance up a heading rising 1 in 3 by means of a hauling-rope passed round a terminal pulley attached to an upright post near the face. Deceased had removed the barhook, and was about to signal for the tram to be lowered, when a fall of roof occurred over the sheave-post, which liberated it, and tram, rope, and sheave ran down the heading, and caught deceased before he could reach a place of safety. He received injuries from which he died on June 14.
