He was travelling down a self-acting endless rope brow, and was found crushed on a full tub against a low bar It probably occurred when he reached over to signal (electric) to move the tubs to enable him to pass full and empty tubs which would be opposite each other. At the inquiry it was advised to have the brow enlarged.
