Deceased had placed a tram of coal on the upper deck of the cage, and was going to signal it up to enable him to load the lower deck when a full tram ran towards him and knocked him into the shaft. He had evidently forgotten to secure the tram when he removed the other from in front of it. It was a mouthing in the shaft. A gate has since been placed there which will prevent a recurrence.
