Inspection made. Deceased was engaged repairing the injector on the loco. when the brakesman lowered two railway waggons towards the screens and as there was very little clearance for the waggons to pass the loco at that point deceased got fatally crushed between the loco and the waggon passing. The brakesman was on the opposite side of the waggons and was not aware deceased was in the way.
