Inspection made and inquest attended. Deceased was in the bottom shunt in the downbrow when he was struck by a lump of coal coming through the tub, caused by the haulage rope breaking in the capping, and the trailer behind the journey of full tubs throwing the tubs off the rails. He was fatally injured, and succumbed to the injuries received June 5, 1906.
