- A self-acting incline with a gradient varying from 1 in 7 to 1 in 9 and about 80 yards long ran loaded tubs both from the top and from a point about 23 yards down where there were flat sheets on to which the loaded tubs were run from the side. The trailer or putter runs his tub on to these sheets, scotches both pairs of wheels and twists it into position on the incline two full tubs are then run down from the top of the incline to the point and the scotched tub is attached to them and the three tubs then run down from the top of the incline to the point and the and other two men were down the incline and when the they passed the intermediate level no full tub was there, but directly afterwards one was brought out and scotched in the usual way, was placed on the incline and an empty tub taken away. The full tub in some way ran amain down the incline, the rope was not running and no explanation was forthcoming why it did not remain at rest. Deceased was caught by the tub near the foot of the incline which was provided with the requisite manholes on the right side of the road, and as he was blind in the right eye this may have prevented him seeing them. It was arranged to have a fixed chock at the intermediate level to prevent runaways.
