- Died Oct. 28. Deceased with the sets to within 30 yards of the shaft and occasionally assisted the onsetter there but was not doing so on the day of the accident. The shaft is 18 fathoms deep to the seam in which deceased worked but sunk other 5 fathoms to a lower level which was being opened out by hewers and deceased was in the act of leaning over a crossbar fencing the shaft at the upper seam to converse with them. There was only one cage in the shaft and the onstter signalled it to the surface empty and the engineman lifted it away but finding it was empty set it back again as there were no empty tubs at the surface. After it had gone for five fathoms it came in contact with deceased’s head as he leaned over the crossbar.
