Fall of roof in the "Big Vein" at a "Barry" face. Thinking the roof was good, he appears to have knocked out a pair of flats in order to move the rails forward, and having done so he intended to reset them. He was however mistaken, as the fall occurred in the shape of a bell with smooth top and caught him under it.
