Deceased was engaged as taker-off on the empty side of the cage, on the lower deck, at the pit bottom. After the cage had been signalled away by the hooker-on, deceased appeared to have attempted to adjust a tub on the cage, when it was raised, and he was lifted and crushed between the lower band of the cage and the edge of the upper platform, death being instantaneous.
