- Deceased and his father were ascending an upcast shaft 34 fathoms in depth on a single decked cage worked by a single cylindered horizontal engine. As they approached the surface deceased remarked that they were going too fast. The cage was raised a considerable distance above the upper scaffold and fearing that it was to be overwound the father leaped off, fell upon the roof of the pithead house and rolled down upon the scaffold. Deceased jumped or fell from the cage and landed upon the pavilion end of the pithead house rolled off and fell down the shaft and was killed instantly.
