He went to the top of an elevator in connection with a coal washer, and was caught by one of the buckets as he was leaning over a tank. The machinery should have been stopped before the boy was allowed to go to the top of the elevator. The management were not aware that he was allowed to go at all, but the man at then bottom appears to have allowed him to do so to oil the machinery.
