Cowburn was employed emptying slack wagons on a siding, when a trapdoor fell from a height of 26 feet and fatally injured him. The door was 24 inches by 18 inches by 3 inches, and was part of the staging at the pit brow. It had been frozen and, when being wrenched up with a crowbar, the door fell through the opening Cowburn was thought to be working elsewhere at the time.
