An overwind had occurred causing one of the fence gates to fall down the shaft. Deceased was going down the shaft to get the gate when the cage stuck at the bottom end of the spear rods. He appears to have jumped out and attempted to reach the inset at surface level and failing, he fell to the bottom of the shaft, a distance of 42 yards.
