Fell from cage while ascending. Deceased had been accustomed for many years to occasionally clear out dirt from water rings in the shaft usually aloe. On this occasion it is believed he going up the shaft in cage for that purpose and fell out. There was absolutely no clue as to how he fell out or where he fell out. The cage was being raised very slowly and was drawn to the top had not stopped in the shaft.
