Inspection made and inquest attended. When the jigger was lowering a full tub from a break to top of jig he failed to place a wooden sprag in front of tub and the full tub rushed down the jig crushing the deceased when passing at the time the bottom of jig. It was recommended at the inquiry that a stop block he placed at top of jig.
