- To enable him and his comrades to proceed with their work of fitting byatts in Rose Heyworth shaft from the three-quarter landing to the bottom (old coal) landing it was necessary to put a lengthening rope on to the winding rope. With the intention of passing the lengthener up through the cage floor for attachment to the winding rope he stepped forward as he expected on to the doors covering the shaft at the three-quarter landing which were supposed to be closed but which unfortunately proved not to be so with the result that he fell (60 yards) to the bottom of the shaft.
