The accident happened in the Thick coal. While deceased was placing a stone against the end of his tub for the purpose of securing it, on a gradient of 1 in 12 a slab of the "stone coal" fell from two unseen slips in the roof and fractured his skull, killing him instantaneously. The roof had been examined and tested by the Fireman 15 minutes previously, and found to be apparently good and sate.
