Deceased was filling a tub at a long wall face in the Shallow seam when a large piece of roof coal fell, and his head was crushed between the tub and the coal, causing fractures of the jaw and other injuries, from which he died on September 19th. The fall was triangular in shape, both in plan and section, and came off by the smooth edge of some intrusive, rock running in the roof coal. Four trees were, reeled out by the fall.
