- While a set of 48 loaded tubs was be being drawn up a road rising 4 inches per yard by a main rope the coupling between the fourth and fifth tubs broke and the set ran back and crushed deceased who was found in a manhole. A bull bogie runs at the end of the set and has two horns which drop a catch the ground if it runs back but on this occasion one of the horns caught and displaced a roller and they did not grip so soon as usual. The couplings are loose and consist of two end links of 1 inch iron and three smaller intermediate links of 7/8inch iron and it was one of the smaller links that gave way at a bad weld. The Local Inspector reported, 'We consider it pure accident.'.
