- Deceased was shovelling sand from a level about 3 feet long by 2 feet wide and 3 feet deep situated close to a face of sand and gravel which was about 12 feet in height. The face had been undermined to a depth of about 4 inches. A quantity of sand and gravel fell from the face without warning and buried him in the hole. He was got out in about ten minutes, but he was dead. His ribs had been badly crushed and driven into his lungs. He had been repeatedly cautioned to maintain more of a slope on the face.
