- He had been working the haulage engine where this occurred during the four days before the accident. Owing to too much slack the rope got between the cog and spur wheels. To release it he worked with a loose drum which resulted in several loose coils on and under the drum. In order to take up this slack the tail rope was attached to the main rope and he started the engine suddenly at a rather high speed. Before he was able to regulate it the rope appears to have raised one of the coils above the drum and it crossed or made a loop and falling over, lassoed deceased around the neck and drew him on to the drum, dislocating his neck.
