- The automatic gates protecting the shaft at the pithead level were out of order and the deceased pinned them up. He had emptied a hutch and intended to put it into the cage, but he, for some unexplained reason, pushed it in at the side the cage was not at and he fell with it on to the top of the other cage, a distance of 462 feet. If the gates had been in working order the accident could not have occurred.
