- Deceased at the end of a shift got on the cage with ten other men. He stood in an outside rank, but at least 2 feet from the edge of the deck. Shortly after the cage commenced its ascent a fireman, standing immediately behind him, noticed him raise his right hand, in which he was carrying two lamps to his head, at the same time dropping one lamp. Almost instantaneously he loosed his grip of the hand-bar with his other hand collapsed on to the floor of the cage, rolled out into the shaft, and fell a distance of about 300 yards. The cage was provided with a hand-bar at each side, but no fencing bars at the ends.
