- Deceased was a rope-rider on a main and tail haulage road, and was riding behind a set of 16 full tubs, when in going up the steepest part of the road, the coupling chain between the second and third tubs broke. The drag failed to hold the 14 tubs which ran back, and deceased, who appeared to have run down the hill some distance in front of the tubs, was caught and killed. The coupling broke at a bad weld.
