Deceased's duty was to pull the empty tubs off the middle deck of a three-decked cage. As the cage descended, he attempted by mistake, to pull a tub out of the bottom deck, and he fell into the shaft, where he was fatally crushed between the cage and the floor of the middle deck landing.
