- Deceased attended to a Pumping Engine, placed alongside the Winding Engine, during his shift both being in the same room. He appears have noticed that the binding bolt required screwing up, so without stopping the Pumping Engine he opened the fence rail and went inside to do this. No one witnessed the accident, but apparently while tightening the bolt he slipped, and was caught both by the flywheel and pump crank, and was killed. He was disobeying instructions in repairing machinery in motion.
