At face of working place from a fault on left side and from a slip on right side, roof taken down to within 9 feet of coal face, a rearing sprag against the roof, whilst driving a wedge into the coal in a stooping position roof fell pushing out the sprag. A straight up prop together with the sprag would have saved the life. The fault and slip urgently called for a prep under. Weight of stone 6 cwt.
