Whilst a train of wagons was being lowered by hand into position at the main coal belt, deceased was standing on a short ladder that was rested against one of the screen supports. The sleeve of his coat was caught by one of the wagon door collars and his body drawn through a 5 inch space between the wagon side and the support was. It was not part of his work to be on the ladder.
