Deceased was assisting a stallman to enlarge a return airway. They had removed a setting of timber and had wedged down a part of the roof preparatory to fixing a new bar, and while deceased was clearing away the last few stones from the floor a piece of roof 4ft. 6ins. and 2ft. 4ins. by 17ins. thick fell suddenly upon him from two unseen slips, causing injuries from which he died on the following day.
