Deceased was killed by a fall of roof in a jig road which the fireman had travelled and inspected a few minutes previously, and found to be apparently safe. Deceased had travelled about 60 yards up the jig when the signal was given and the tubs set in motion, and shortly afterwards, the jigger healing the fall, stopped them and went down to ascertain what had happened. He found deceased under the jag, but before he could be extricated, more ground fell and dislocated his neck. Both sets of tubs had passed the site of the fall and were standing intact upon the rails. There was ample space between the tubs and the timber and there was nothing in the tubs to cause the timber to be displaced, nor was there any cavity above the timber before the fall occurred. Some of the timber was broken, but four settings, not exceeding 3 feet apart, were reeled out, and the roof over a length of 17 feet by 12 feet wide, including part of the side, had fallen, exposing numerous slips in tile roof. It was surmised that simultaneously with the starting of the jig, the roof began to weight, and then deceased took refuge in a man-hole until the tubs passed him and then endeavoured to get to a place of safety, only to be caught by the fall.
