After putting a driving belt upon a pulley deceased was returning along a gangway underneath the pithead scaffold. At a point where the clear height was 4ft 2 in his head apparently came against an overhead beam he overbalanced and fell to the ground a distance of 16 feet striking the end of a truck as he fell. He sustained injuries which resulted fatally 10 hours after.
