- Deceased was at work in a heading 8 feet wide which was crossing two converging downthrow faults. He was shearing or nicking at the face, when presumably he heard the roof working and made to run back. A mass of bass 8 feet by 6 feet by 3 feet, fell from between the two faults, reeled out a prop and caught him as he passed underneath he was severely crushed internally and died about two hours afterwards. The place had been examined by the fireman and the roof tapped about an hour before the accident and it appeared to him to be safe.
