Deceased was taking an empty tub along the level, when the tub left the rails, knocked out a tree supporting a bar and let down a large piece of rock, the end of which struck him on the head and knocked him down with his face on the ground. He was got out, but died on reaching the surface. Deceased was seen on the chains when he left the station and was undoubtedly riding the chains when the accident occurred, this being a breach of Special Rule 91. This is a dangerous practice, unfortunately far too common in this district.
