- Deceased was crushed by a runaway full tub which bad been bumped on to the brow by another tub. The stop block provided was not put in, nor was the stang prop fixed in position. The tub had been left in a dangerous position too near the brow. As an additional safeguard a drop gate was provided near the bottom of the brow for which deceased was responsible, and this also was not in position. Deceased was so seriously injured that he died the next day.
